Activity Waiver and Release of Liability

Sky Entertainment / Sky Classes is fully insured, and completion of this waiver is required for all participants in our programs, including classes, workshops, private lessons, camps, open practices, and events

Please fill out the following form for the participant

1. Assumption of Risk

I understand that participation in physical activities offered by Sky Dance Studio / Sky Entertainment involves inherent risks.

These activities include, but are not limited to, aerial silks, acrobatics, and dance, which may involve working at height, use of specialized equipment, physical exertion, and complex movements.

Risks include, but are not limited to, falls from height (in aerial activities), slips, trips, collisions, equipment failure, strains, sprains, overexertion, and other injuries that may be serious in nature.

I voluntarily choose to participate (or allow my child to participate) and assume all risks, known or unknown.

2. Release of Liability

In consideration of participation in any activity organized or operated by Sky Dance Studio / Sky Entertainment, I hereby release and hold harmless the company, its owners, instructors, assistants, staff, contractors, and any associated venues from any and all claims, demands, or causes of action arising from injury, loss, or damage.

This includes any claims arising from negligence, equipment use, instruction, or any other aspect of participation.

3. Fitness to Participate

I confirm that I (or my child) am in good physical condition and able to safely participate in these activities.

I understand that it is my responsibility to consult a physician if I have any concerns and to inform instructors of any injury, condition, or limitation prior to participation.

4. Safety and Conduct

I agree to follow all instructor directions and safety guidelines at all times.

I understand that failure to follow safety instructions or engaging in unsafe or disruptive behavior may result in removal from class or activity without refund.

5. Emergency Medical Consent

I authorize Sky Dance Studio / Sky Entertainment staff to provide basic first aid and to contact emergency medical services if necessary.

I understand that I am financially responsible for any medical treatment or care received.

6. Personal Property

I understand that Sky Dance Studio / Sky Entertainment is not responsible for lost, stolen, or damaged personal belongings.

7. Photo and Video Release

I acknowledge that photos and/or videos may be taken during participation for promotional and marketing purposes, including social media and website use.

If I do not consent, I will notify the studio in writing prior to participation.

8. Indemnification

I agree to indemnify and hold harmless Sky Dance Studio / Sky Entertainment, its staff, contractors, and associated venues from any claims, damages, or legal expenses arising from participation.

9. Governing Law

This agreement shall be governed by the laws of the Province of Nova Scotia.Any disputes shall be resolved in the courts of Halifax, Nova Scotia.

10. Acknowledgment of Understanding

I confirm that I have read and fully understand this waiver.

I acknowledge that by agreeing, I am voluntarily giving up certain legal rights, including the right to sue.
